Hi!
Need urgent advice/help on Medical Surveillance.
My husband received a medical surveillance form which was signed and stamped at the port of entry in Toronto.
Unfortunately, we have lost the only copy we have of this document and we are required to visit a health centre within 30 days as per instructions from IRCC.
What can be the next step we can look at in order to proceed?
If someone has gone through the process and tell us what documents do they ask for at the first health checkup visit.
IRCC is not taking calls due to the strike.
We also thought it would be better to get the Health Card and then go as the fees will be waived off.
Please advice.
